In CCDS's ongoing efforts to simplify and streamline your experience during the back-to-school season, we've digitized several required parental preference forms. Now, you can easily complete the following forms in ParentVUE:

1. Media Center Access and Book Check-out. For more information regarding this form, please CLICK HERE.

2. Consent for School-Based Healthcare Services. If this form is not completed, the clinic will not be able to provide services to your student.

3. Intent to Utilize Bus Transportation (this is not the same as bus registration)

Accessing and completing these required forms online will ensure a smoother and more efficient process before the first day of school on August 11th. Please log in to ParentVUE to complete these forms at your earliest convenience.
